📋 Tanggung Jawab Pekerjaan

Assess and evaluate the needs of individuals and families to develop comprehensive case plans.,Coordinate and manage case management services, including connecting clients with appropriate resources and services.,Monitor and evaluate the progress of case plans, and make adjustments as needed.,Provide case management support to individuals and families, including advocacy, counseling, and referrals.,Collaborate with a multidisciplinary team to ensure holistic and integrated service delivery.,Maintain accurate and up-to-date records of case management activities and client information.,Participate in staff meetings, training sessions, and team-building activities.,Promote and advocate for the rights and well-being of clients within the community.

🎓 Kualifikasi & Syarat

Bachelor's degree in Social Work, Psychology, Social Sciences, or a related field.,Minimum of 5 years of experience in case management, social work, or a related field.,Strong understanding of case management principles, practices, and ethical standards.,Excellent communication, interpersonal, and negotiation skills.,Ability to work independently and as part of a team in a fast-paced environment.,Proficiency in English, both written and spoken.,Knowledge of local laws, regulations, and community resources.,Valid driver's license and reliable transportation.
